Report an Incident, Complaint and Feedback

Swan Living takes the safety and wellbeing of residents, staff, visitors and neighbours seriously. Use this page to tell us about an incident you experienced, witnessed or became aware of in connection with one of our properties or services.

Immediate danger or emergency

Call 999 if someone is in immediate danger, a serious crime is taking place, there is a fire, or urgent medical assistance is needed. Submit this form afterwards only when it is safe to do so.

What You Can Report

  • Anti-social behaviour, harassment, threats or violence.
  • Accidents, injuries or near misses.
  • Safeguarding concerns, exploitation or abuse.
  • Damage to a property or communal area.
  • Fire safety, electrical, gas or other health and safety concerns.
  • A serious breach of house expectations.
  • Missing person or welfare concerns, following the correct urgent procedure.
  • Any other event that may require investigation, support or action.

Can I Report Anonymously?

You may choose not to give your name. Anonymous reports will still be reviewed, although limited information can make it harder to investigate, ask follow-up questions or provide an outcome. Swan Living will handle personal information in line with its privacy notice and safeguarding responsibilities.

Confidentiality and Fairness

Swan Living will protect information as far as possible, but cannot promise complete confidentiality where there is a safeguarding concern, risk of serious harm, legal obligation or need to investigate fairly. Reports are considered objectively and no conclusion is reached until relevant information has been reviewed.

1. Reporter Profile
Required to properly direct and track the report internally.
Optional; please specify your preferred communication method and safe times to contact you.
2. Incident Details
Specific entries are encouraged; text values allow “approximate” and “ongoing” parameters.
Select all tracking modules that apply:
Provide full names or descriptions, only where known and strictly necessary.
Detail exactly what happened, where, the chronological sequence of events and immediate impact.
Reminder: If active injuries or structural fire risks exist, call 999 immediately before reporting.
3. Interventions & Documentation
For example: informed on-site staff, moved residents to safety or sought direct medical help.
Collect and list contact details only with an appropriate, valid basis.
Warning: Upload relevant photos or documents; do not put yourself at risk to obtain structural evidence.
